Facility Evaluation Report

Licensing Program Analyst (LPA) Holly Williams arrived unannounced to conduct a collateral visit. LPA Williams met with caregiver Isaac Ratu and explained the purpose of the visit. Bio Ratu is the licensee and LPA Williams talked with them on the phone and they gave permission for Isaac Ratu to sign the report.

This investigation consisted of interview with R1. In an interview, R1 stated that he did not receive a written eviction notice and that Drusilla Uwoghiren the administrator for Garden of Joy told R1 that R1 had to leave. R1 said they want to drop all allegations and doesn't remember anyone yelling at anyone. R1 stated there was caregiver that lived at Garden of Joy but they quit and

No deficiencies were cited regarding the above allegation. An exit interview was held and a copy of this report was left with Isaac Ratu.
