The inspector’s account

On 7/2/2025 LPA Hughes conducted a follow-up facility visit, during facility observation EXIT signage located on the fire door was observed and properly labeled as an Exit, Fire door remained closed.

Based on the information obtained, there is no evidence to support the allegation, therefore it could not be corroborated at this time.

Allegation: Staff isolate residents in their rooms

It was alleged that staff isolate residents in their rooms. This investigation consisted of facility observation, and interview with residents. On 5/15/2025 LPA Pang Lee conducted a facility visit, residents in care were freely moving in and out of their rooms and engaging in interactions outside their rooms. On 7/2/2025 interviews with 2 out of 2 residents indicated that residents have no concern with staff isolating residents in their rooms. LPA Hughes observed 2 additional residents watching television outside of their rooms in the common area. Based on the information obtained, there is no evidence to support the allegation, therefore it could not be corroborated at this time.

Allegation: Staff did not provide assistant to resident in a timely manner

It was alleged that staff did not provide assistant to resident in a timely manner. This investigation consisted of facility observations, and interviews with residents. On 5/15/2025 LPA Pang Lee conducted a facility visit, and observation during the visit reflected that facility care staff were actively engaged in assisting residents with medication administration, showering, and preparing meals. On 6/17/2025 LPA Shakaricka Hughes, conducted a follow-up facility visit, and observed facility staff assisting residents with preparing lunch, and duties that included care needs of the residents within resident bedrooms. Resident interviews indicated that residents have no concerns with facility staff not being able to assist residents in a timely manner. Based on the information obtained, there is no evidence to support the allegation, therefore it could not be corroborated at this time.

The investigation revealed the preponderance of evidence standards have not been met; therefore, the above allegations are found to be UNSUBSTANTIATED . A finding that the complaint allegations are UNSUBSTANTIATED means that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred.
