Facility Evaluation Report
On July 24, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct an annual inspection utilizing the care tool. LPA met with Administrator, and explained the purpose of the visit.
LPA and Administrator conducted a tour of the facility to ensure the health and safety of clients in care. LPA observed five private clients room, two bathrooms, kitchen, laundry room, and the common areas. LPA observed no clients present as they were all in day program. LPA observed medications, sharps and toxins to be locked and secured. Resident files were found to be complete and current. A review of staff records indicates that all facility staff have received criminal record clearances and/or are associated to this facility. LPA observed facility to have ample supply of perishable and nonperishable foods present. LPA observed facility to be clean, safe and in good repair.
LPA and Administrator discussed pending Administrator Certificate. Administrator stated all documents are to be mailed today. Additionally, LPA and Administrator discussed completing age exceptions for clients in care.
Inspection tool completed, and no deficiencies cited.
Exit interview conducted and a copy of report was provided.
