Facility Evaluation Report

Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 11/04/25 to do case management visit . LPA met with staff, DSP, Matthew Wilson and explained the purpose of the visit. LPA and staff spoke with Program Administrator, Latisha Woodson (LW) via phone who gave permission to conduct today's visit with staff as LW was not able to come to the facility.

Department followed up on Incident Report and SOC 341 sent by facility on 10/27/25 for date 10/24/25 regarding resident, R1 and staff , S1. Facility notified long term care ombudsman (LTCO) , R1s responsible party, ALTA Regional Center and other required agencies regarding this incident.

Based on gathered information, it has been concluded that facility did not safeguard resident's Personal Rights as staff S1 was verbally abusive to resident ,R1 during care while presence of other residents and staff. Per Title 22 Regulations, Citation-A was issued for Residents Personal Rights ( 80072) as listed on LIC809-D.

Exit interview conducted ,appeal rights and copy of the report left at facility. Civil penalties shall be assessed if facility does not comply with POC requirements which were issued today.
