Facility Evaluation Report
On 04/15/25, Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility to conduct a Case Management visit regarding an incident that occurred on 03/14/25 for resident R1 regarding medication error. LPA met with Assistant Administrator, Jesus Ramirez and explained reason for visit.
Alta California Regional Center Special Incident Report submitted by facility on 03/15/25 to CCL stated that at approximately 07:14PM, staff discovered that resident, R1 did not receive their one (1) pill of Hydroxyzine 50mg which was scheduled for 5PM. SIR stated that staff prepare the medication, but it was not administered to R1 as prescribed by R1s physician.
Facility notified R1s physician, CCL, ALTA and other agencies regarding this incident.
Department issued Technical Advisory to the facility regarding this incident during today’s visit.
The report was reviewed, and a copy of this report was left at the facility.
