Facility Evaluation Report

Licensing Program Analyst (LPAs) Lavinia Muscan and Talwinder Bains arrived at the facility unannounced on 05/05/2026 to conduct a case management inspection to follow up on an incident on 04/24/26. LPAs met with Autiana Brown Assistant Admin and explained the purpose of the visit.

Facility submitted an incident report (LIC624) and an SOC341 to the department on 04/24/26 involving 2 staff members and a client.

During today visit, LPAs interviewed R1. After reviewing the incidents reports and information gathered, it has been determined that facility took appropriate measures to address the issue. The department will follow up as needed.

No citations were observed or cited per Title 22 Regulations.
