Facility Evaluation Report
On 09/22/25 at 11:00AM, an office meeting was conducted with Sacramento Regional Office via Microsoft Teams. The purpose of this meeting is to discuss the incident and SOC341 related to resident, R1 and staff, S1 for date, 09/17/25 . Present in the meeting were Licensing Program Manager (LPM) Laura Munoz, Licensing Program Analyst (LPA) Talwinder Bains, and Administrator, Kari Pallick and Senior Clinical Director, Sarah Jacobs .
Department discussed the incident and SOC341 with facility staff. The department told staff that after reviewing documents and interviews, it has been concluded that department will not issue any citations or any corrective action to S1 and to the facility due to this incident.
No deficiencies were cited per Title 22 Regulations. Exit interview conducted. Informal meeting concluded.
A copy must be signed and returned to the Department .
