Facility Evaluation Report

On 6/29/26 at 10AM, a virtual office meeting was held with Sacramento North Regional Office using Microsoft Teams.

Present in the meeting was: Licensee, Etelka Ginga and Matt Williams,Licensing Program Manager(LPM) Laura Munoz, and Licensing Program Analyst (LPA) Talwinder Bains.

The purpose of today's meeting was to discuss a pending change in the facility's ownership (CHOW).

Licensee discussed how they are working with potential buyers and working on change of ownership (CHOW) option at this time. Licensee stated that if for some reason the CHOW is not approved,they will close the facility. Should the facility choose to close, the discussion included the facility’s requirement to issue a notice to all residents and their responsible person(s) advising of the pending change in ownership, and how a copy of that notice needs to be submitted to the Department within (5) days of its issuance. Licensee understands that they are responsible for residents care until CHOW or closure of the facility get completed.

Licensee will reach out to department for further information regarding this matter.

A copy of this report was emailed to Licensee, following today's meeting.

A signed copy will be returned to the LPA.
