Facility Evaluation Report

Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to follow up on a complaint investigation. During the course of that investigation, LPA Moleski discovered an unrelated deficiency. LPA Moleski met with house manager Brittney Chaves and explained the purpose of the visit.

While conducting interviews regarding the complaint allegations, LPA Moleski was informed of a previous incident of psychological and/or verbal abuse at this facility which was not addressed by the Community Care Licensing Division at the time. LPA Moleski reviewed an incident report from this facility which described a staff member's (S1's) actions while being recorded by another staff member on 7/4/22. According to the incident report, the audio recording described S1 being "verbally abusive" toward a resident (R1). S1 can be heard on the recording telling R1 "This is what you get. This is what you wanted, right? This is what [R1] wants, a cold shower. I'm going to give [R1] a cold shower," according to the incident report. The incident report states that the audio implied S1 was giving R1 a cold shower as punishment for their behaviors.

At the time the report was made, CCLD did not follow up to address the issue.

This facility is hereby cited per 22 CCR Section 80072(a)(3). An exit interview was held with facility Hanif Khan over the phone. Appeal rights and a copy of this report were left with Chaves.
