Facility Evaluation Report

Licensing Program Analyst (LPA) Jason Lund arrived at the facility unannounced to do a case management visit. LPA Lund met with staff and later with Administrator Beatrice Clark.

During LPA Lund visit LPA Lund spoke with all residents in care. During the interview process LPA Lund spoke with Resident (R1) who stated that R1 didn’t want to get out of the bed. LPA Lund did not observe a Hoyer lift or a wheelchair to help him R1 get out of bed.

LPA Lund issued citations and civil penalties during todays visit which are attached. Exit interview and copy report, appeal rights were left at the facility.
