The inspector’s account

On August 25, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived at the facility unannounced to conduct a required annual inspection utilizing the inspection tool. LPA met with Staff and explained the purpose of the visit.

There are four residents in care with no residents on hospice services. LPA was informed a death report should be faxed to Licensing shortly as a resident expired hours prior to LPA's visit.

Today's inspection, LPA and Staff conducted a tour to ensure the health and safety of residents in care. Areas toured included but not limited to: kitchen, laundry room, residents room, bathrooms, and the common areas. LPA observed facility to have the required postings posted in the common areas. LPA observed facility to have at least two days of perishable and seven days of nonperishable food available at the facility. LPA observed facility to be at a comfortable temperature of 72*. LPA observed carbon monoxide present at the facility and in working condition. LPA observed sharps, toxins and medications to be locked and inaccessible to residents in care.

File review conducted for four residents records and three personnel records. LPA observed the files to be organized with the required documents completed and present. Fire drill records revealed facility completed drills on March 12, 2025 and June 4, 2025. Emergency Disaster Plan was recently updated and reviewed on June 3, 2025. LPA observed active liability insurance present on file, compliance to Health and Safety Code.

CARE tool was completed and found facility to be in substantial compliance. No deficiencies cited.
