Facility Evaluation Report
On February 5, 2026, Licensing Program Analyst (LPA) Licensing Program Analyst (LPA) Cassie Yang arrived at the facility to conduct an annual inspection utilizing the CARE tool. LPA met with Administrator and explained the purpose of the visit.
Today's visit, LPA and Administrator conducted a tour of the interior and exterior of the facility. LPA observed facility to have the required posters posted in the common area. LPA observed residents bedrooms to be properly furnished. LPA observed bathrooms to be equipped with hand bars, personal hygiene supplies, and observed bathrooms to be free of mal odors. LPA observed facility to have ample supply of food available for residents in care. LPA observed exterior pathways to be free of obstructions. LPA observed facility to be in good repair.
File review was conducted for residents records, personnel records, and Emergency Disaster Plan. LPA observed documents to be complete and compliance to Title 22 requirements.
LPA and Administrator discussed obtaining an exception waiver for a resident with unstageable wound, LPA provided Administrator a copy of 87616 Exceptions for Health Conditions.
CARE tool was completed and found facility to be in compliance at this time.
Exit Interview and copy of report provided to Administrator.
