Facility Evaluation Report

On October 7, 2025 at 2:00 PM, a virtual office meeting was held with Sacramento North Regional Office using Microsoft Teams.

Present in the meeting was: Licensee Che Chang, Administrator Mulunesh Terefe, Licensing Program Manager (LPM) Troy Ordonez, and Licensing Program Analyst (LPA) Cassie Yang.

The purpose of today's meeting was to discuss Licensee's intent to change facility's ownership.

Resubmitting 60 day notice if new application is not approved by effective date.

Administrator's responsibility of day to day operation.

Licensee's accountability and responsibility until new application is approved.

Facility compliance to ensure all staff and individuals residing at the facility are fingerprint cleared and associated to the facility roster on Guardian.

LPM emphasized how communication needs to be ongoing with the Department during the ownership transition, and for any other significant changes, such as a change to the property ownership, and the facility's first point of contact is the assigned LPA.

At this time, Licensee is to submit an updated LIC 500 and LIC 308.

No deficiencies cited during today's meeting.

A copy of this report was emailed to Licensee. A signed copy will be returned to the LPA by close of business day.
