Facility Evaluation Report
On October 1, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management visit. LPA met with staff who then contacted Administrator who arrived shortly to the facility. LPA spoke with Licensee via telephone. Licensee arrived at the facility towards end of visit.
LPA, Licensee and Administrator discussed the pending change of ownership.
Today's visit, LPA provided Administrator a copy of 87211 Reporting Requirement and 87224 Eviction Procedures.
Additionally, LPA informed Licensee and Administrator an office meeting may be scheduled to discuss this matter further. No deficiencies cited.
