Facility Evaluation Report

Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management visit. LPA met with Administrator and explained the purpose of the visit.

Today's visit, LPA and Administrator discussed the facility's plan of change of ownership. LPA and Administrator discussed issuing residents, responsible parties and LPA a copy of a 60 day change of ownership notification. Additionally, LPA and Administrator discussed hospice waiver, required staff training, required resident and staff documents.
