Facility Evaluation Report
Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection tool. LPA.met with Administrator and explained the purpose of the visit.
Today's visit, LPA and Administrator conducted a tour of the interior and exterior of the facility. Areas toured included but not limited to: five residents bedrooms, bathrooms, kitchen, office, laundry and the common areas. LPA observed facility to be clean, sanitary and in good repair. LPA observed medication, toxins and sharps to be locked and inaccessible to residents in care. In areas toured, no immediate health, safety and personal rights violation was observed.
File review conducted for three residents. LPA observed facility has three Administrators working at the facility.
Emergency Disaster Plan was observed and recently updated in March 2025. LPA observed documentation of fire drills conducted. CARE tool completed and found facility to be in substantial compliance.
As a result of today's inspection, no deficiencies cited.
Exit interview conducted and a copy of report was provided.
