Facility Evaluation Report

LPA Hiratsuka, conducted this visit to check on the status. LPA was informed the facility was closing last month. LPA received the license at the office with a notice stating the facility was closed effective April 1, 2026, but then received a phone call stating the facility was going to stay open until further notice. There are no residents in the facility.

Today, LPA was informed the facility is officially closing on May 1, 2026. LPA requested a written notice from the licensee to be sent stating the facility closure date.

If the licensee wishes to operate a Residential Care Facility for the Elderly in the future she will have to apply for a brand new license.
