Facility Evaluation Report
Licensing Program Analyst (LPA) Hiratsuka arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection care tool. LPA met with staff, and explained the purpose of the visit. Staff then contacted Administrator who arrived to the facility.
Today's census is one resident in care with no residents on hospice services.
LPA and Administrator conducted a tour of the facility to ensure the health and safety residents in care. Areas toured included but not limited to: resident bedrooms, bathroom, kitchen and the common areas. LPA observed the facility to have the mandated compliance posters. LPA observed facility to have two days of perishable and seven days of nonperishable foods. Facility was observed to be clean, free of odor, sanitary and in good repair. LPA observed no immediate health safety and personal rights violation.
CARE tool completed and as a result of today's visit, no deficiencies cited.
Exit interview conducted and a copy of report was provided.
