Facility Evaluation Report

On April 28, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection care tool. LPA met with staff, and explained the purpose of the visit. Staff then contacted Administrator who arrived to the facility.

Today's census is two residents in care with no residents on hospice services.

LPA and Administrator conducted a tour of the facility to ensure the health and safety residents in care. Areas toured included but not limited to: resident bedrooms, bathroom, kitchen and the common areas. LPA observed the facility to have the mandated compliance posters. LPA observed recently services fire extinguisher present, serviced March 7, 2025. Facility at a comfortable temperature of 73*F. LPA observed facility to have two days of perishable and seven days of nonperishable foods. Facility was observed to be clean, free of odor, sanitary and in good repair. LPA observed no immediate health safety and personal rights violation.

LPA reviewed Emergency Disaster Plan to be recently updated July 2024. LPA conducted a file review of two residents records and observed the required documents in file. File review of staff on shift, LPA observed S1 to have documentation of training and fingerprint clearance.

CARE tool completed and as a result of today's visit, no deficiencies cited.
