Facility Evaluation Report
Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a case management inspection related to a recent incident report (LIC624) submitted to the Department on December 8, 2025. LPA met with Rowena Tirasol, Lead caregiver, and stated the reason for today's inspection. The Administrator, Rosana Mead, arrived at 3:20 pm. There is currently (1) resident under hospice care. LPA observed (2) residents present in the common area and (1) resident resting in their room.
LPA reviewed the LIC624 which states resident (R1) went to the emergency room on December 5, 2025 after vomiting and then passing out around 11:30 am. The hospital discharge paperwork shows (R1) was treated for syncope and an elevated white blood cell count, and returned to the care home on December 8, 2025. A new medication was prescribed for (Senna) and (2) medications, Chlorthalidone 25mg and Lisinopril 20 mg were discontinued. (R1) had a follow up phone call appointment with their physician on December 9, 2025, but the responsible per son was not able to attend, so it will be re-scheduled. The Administrator indicated that (R1) has been resting and doing well, without any further vomiting, since returning. LPA observed (R1) to be resting in their room.
Also discussed was that resident (R2) passed on November 24, 2025, while under hospice care. The administrator and death report indicate (R2) had been declining and only took liquids for the last two weeks. The lead staff and administrator stated (R2) had been previously restless over the last (2), but was peaceful for the last (4) days prior to passing. Hospice staff was visiting times weekly, and then visited more when the facility contacted them. LPA and the administrator toured the interior of the facility and did not observe any health and/or safety risk to the residents in care. There is sufficient food.
The facility took appropriate action in sending (R1) out for prompt medical attention. There are no deficiencies issued in the report . Exit interview. Copy of report provided.
