Facility Evaluation Report

Licensing Program Analysts (LPA) Kevin Mknelly Ivan Avila arrived at the facility unannounced on 4/6/23 to conduct a Annual Inspection. LPAs met with Executive Director and explained the purpose of the visit.

LPA toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, 9 resident bedrooms, bathroom, kitchen, laundry room, and exterior. In the areas toured no immediate health, safety, or personal rights violations were observed.

LPAs conducted staff file reviews and were unable to complete Resident file reviews during this visit. LPA Mknelly will return to complete the inspection.

No deficiencies are being cited as a result of todays inspection.
