Facility Evaluation Report
Licensing Program Analyst (LPA) Lavinia Muscan arrived unannounced at the facility to conduct a case management visit regarding an incident the Department received. LPA met with Administrator (Admin), Eveleth Purugganan, and explained the purpose of the visit.
LPA observed the facility modified the detached garage with walls creating a living/sleeping space for staff. Interviews indicated staff have slept in this space at some point.
The licensee did not obtain proper permits, fire clearance nor approval from the department to use this space for staff.
This space is not approved for living/sleeping at this time. Facility staff shall cease from utilizing this space for living/sleeping arrangements until proper permits and clearances have been approved.
As a result of todays visit deficiencies were observed and cited as indicated on LIC 809-D. Civil penalties may be assessed if facility does not comply with POC requirements issued today.
Exit interview conducted and a copy of the report, LIC809G and appeal rights were left at the facility.
