Facility Evaluation Report
Licensing Program Analyst (LPA) Todd Tryon arrived unannounced at the facility to conduct a required annual inspection. LPA met with Licensee/Administrator Eveleth Purgganan and explained the purpose of the visit.
LPA toured the facility with Mrs. Purgganan.
As LPA arrived, all the clients and some staff were leaving the house to go on a picnic, therefore LPA did not interview clients.
LPA toured the resident bedrooms, bathrooms, kitchen, laundry room, backyard and the common areas. LPA observed facility to be at a comfortable temperature. The home is clean, well-furnished and organized. Food supplies were adequate to meet the requirement of 2 days perishable and 7 days non-perishable. Medications and potentially hazardous items and chemicals were locked. Smoke detectors installed and functional, fire extinguisher charged within the past year. No hazards were noted inside the house or in the yard.
LPA reviewed the CARE Tool with licensee.
File review was conducted for 2 clients, and 3 personnel files. Files include all required documents.
The facility appears to be in substantial compliance with the regulations at this time. No deficiencies were cited at this visit. Exit interview conducted.
