Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Hood arrived at the facility and met with Administrator, Zachary Spain, to follow-up regarding an incident report received by the Department.
Unusual Incident/Injury Report (SIR) dated September 8, 2025 indicated that, on September 7, 2025, resident (R1) left the facility and did not return after check-in hours. Upon R1's return to the facility, R1 was reported to be "off baseline." SIR reports that R1 alleged sexual assault. SIR indicates R1 was sent to the hospital and conservator was notified.
Interviews with Administrator and Campus Administrator, Maritza Vindell, indicated that they were informed following the incident that the hospital did not file a police report and social worker did not refer R1 for medical assessment. Interview with R1 indicated that they have been assaulted since reported incident. Administrators were notified regarding R1's claims made during visit and R1's conservator was notified during visit.
LPA obtained and reviewed documentation pertaining to R1. LPA will conduct a follow-up visit if deemed necessary.
No deficiencies are being cited as a result of today's visit. Exit interview was conducted. Signature on these forms acknowledges receipt of these documents.
