Facility Evaluation Report
On 10/28/2025 Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct a required 1 year annual inspection. LPA met with Administrator Alexei Iovita and explained the purpose of the visit.
LPA and Administrator conducted a tour of the interior and exterior of the facility. Areas toured include but not limited to resident bedrooms, bathrooms, kitchen, laundry room, common areas and the backyard. LPA observed sufficient furniture and lighting throughout the facility. LPA observed there is sufficient food supplies for seven (7) day non-perishable and two (2) day perishable. LPA observed toxins and knives to be locked and inaccessible to residents in care. LPA also observed centrally stored medications are kept locked and inaccessible to residents. Fire extinguishers was last inspected on 10/21/2025. Smoke detectors are current and in compliance with fire safety including carbon monoxide detector. The hot water temperature in the kitchen sink was observed to be 106 degrees Fahrenheit.
LPA reviewed four (4) resident files. LPA reviewed two (2) staff files. A review of staff records indicates that all facility staff has received criminal record clearances and/or are associated to this facility. Staff records reviewed indicated current training completed.
LPA requested Administrator to send updated copies of the following by 11/04/25 to LPA
LIC308- Designation of Administrative Responsibility
No deficiencies being cited during today's inspection. Exit interview conducted. Copy of report and LIC809G provided.
