The inspector’s account

Licensing Program Analyst (LPA) Hannah Rodgers conducted an announced case management visit to cite a deficiency observed during a complaint visit that are unrelated to the complaint allegations. LPA identified herself to, was greeted by, and explained the purpose of the visit to Administrator Omorodion Owens.

During the unrelated complaint visit, LPA observed staff place a locking mechanism on the front door of the facility from the inside. The locking mechanism was placed on the inside door handle and fastened with a lock and key. The front door could not be opened from the outside or inside without unlocking the mechanism around the door handle. Interviews revealed that Client #1 (C1) is an elopement risk and they use the locking mechanism to prevent C1 from eloping from the facility. Therefore, one deficiency is being cited per California Code of Regulations Title 22 and noted on the attached LIC809-D page. Additionally, since the deficiency was related to a fire clearance violation, an immediate Civil Penalty of $500 was also assessed on the LIC421IM.
