Facility Evaluation Report

Licensing Program Analyst (LPA),Abdoulaye Zerbo conducted an unannounced visit to the facility for a health an safety check. The LPA met with Administrator Larisa Madera, and informed her of the purpose of the visit and was granted access.

Community Care Licensing received an incident report indicating that medication for Resident 1 (R1) was concealed in yogurt. During an interview, R1 confirmed their preference for taking medication with yogurt. Additional staff interviews corroborated that R1 requests yogurt when it's time to take their medication. The facility representative informed LPA that R1 has a doctor's appointment on July 29, 2025, to include the yogurt method in the physician's report. LPA requested the facility to share a copy of the updated physician report when available. The Administrator was advised that further investigation is still needed at this time.

An exit interview was conducted and a copy of this report was provided to Administrator Larisa Madera
