Facility Evaluation Report
Licensing Program Analyst (LPA), Abdoulaye Zerbo conducted a visit to the facility for the purpose of a case management. LPA met with the Assistant Administrator Angel Agavo and informed them of the purpose of the visit.
On June 3, 2025, Community Care Licensing received an incident report indicating staff member (S1) smacked client’s (C1) hand and made verbal threats toward C1. During the site visit, the LPA conducted a facility tour and found no health or safety concerns. The LPA also interviewed staff and clients and obtained relevant documentation regarding the incident.
The Assistant Administrator informed LPA that S1 was immediately suspended on May 29, 2025, upon notification of the incident. Furthermore, based on additional information obtained by facility representatives, S1 was terminated effective immediately on June 3, 2025.
The LPA advised the Assistant Administrator that further investigation is required at this time.
An exit interview was conducted, and a copy of this report was provided to Assistant Administrator Angel Agavo
