Facility Evaluation Report
Licensing Program Analyst (LPA) Robert Campbell conducted an unannounced visit to the facility for a case management visit to follow up from the Annual Inspection on 2/19/2025. The LPA was allowed entrance into the facility after five minutes and met with Resident #1 who opened the door. Alexander Busby/Staff#1 came from the back of the facility 3 minutes later appearing that staff#1 just woke up from sleep. LPA informed the Licensee of the purpose of the visit over the phone because they was at Costco about to go shopping. LPA conducted staff record review. LPA found no current staff training since 2024 and that the Administrator stated over the phone that they had not started working on getting an Administrator certificate yet.
LPA toured the facility and did not find any health and safety concerns for the clients in care. Type B deficiencies ( are being cited under failure to have a current Administrator Certificate and not having current training for Staff#1 and lack of supervision by Staff#1 per Code of Regulations, Title 22, Division 6 (87405(a) 87411(c), & 87468.2(a)(4) found on LIC9099D) .
An exit interview was conducted where a copy of this report was discussed with and provided to the Case Manager.
