The inspector’s account

On 12/17/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit. LPA was greeted and granted entry by staff Keith Hill, where LPA explained the purpose of the visit. The House Lead Rachelle Nicholson, and administrator Tyheara Marrow arrived shortly after. At the time of the visit there was zero clients, as they were the day program. Prior to conducting the visit LPA conducted a file a review and observed for the licensing fees had been paid and the governing body was observed to be in good standing. LPA verified contact information for the facility and will make the necessary changes. Below are the observations made during today's visit:

The facility was observed to be clean and clutter free, the medications, sharps and chemicals were observed to be locked and inaccessible to clients in care. The facility was observed to have the required postings, and sufficient food supply (2 day supply of perishable and a 7 day supply of nonperishable food items). The smoke and carbon monoxide detectors were tested and found to be operable. There are no bodies of water or ammunition on the premises. The facility is conducting emergency disaster drills on a monthly basis, with the last drill was conducted on 11/02/25. The fire extinguisher was last serviced on 02/10/25. The hot water temperature was tested and ranged from 113.8 to 136 degrees Fahrenheit. The hot water heater was adjusted during LPAs visit and a caution hot water sign was posted in the applicable restroom.

LPA conducted a file review was conducted of staff and client files. The staff were observed to have the required training including CPR certification. The administrator on record Tyheara was observed to possess a valid administrator certificate that expires on 04/14/26. The facility was observed to have to have emergency supplies food and water. Based on today's inspection there were no deficiencies observed. An exit interview was conducted where a copy of this report was reviewed and provided to Rachelle Nicholson.
