Facility Evaluation Report
On 03/17/2026, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. Administrator Kimberly Mc Intosh arrived to the facility to meet with LPA. The facility has a fire clearance to serve four (4) ambulatory adults ages 18 through 59. During the visit, LPA observed one (1) staff and no clients present. LPA was informed the clients were out in the community attending day program.
LPA toured the facility with Administrator Mc Intosh. The facility is made up of a two-story home with four (4) client bedrooms and two (2) bathrooms along with a kitchen, office, dining room, living room and attached garage. Indoor and outdoor passageways are free of obstruction. No bodies of water were observed on the premises. Client bedrooms had the required bedding, furniture and lighting. Client bathrooms had soap, toilet paper, and paper towels readily available. LPA toured the kitchen and observed the facility met Departmental requirements for a two-day supply of perishable foods and seven-day supply of nonperishable food items. Medications are centrally stored and secured in a locked kitchen cabinet. One of the smoke alarms/carbon monoxide detectors was tested and LPA heard it to be operational. LPA also observed charged fire extinguishers mounted throughout the facility which were last serviced on 02/03/2026. The facility has an emergency supply of food, water, Personal Protective Equipment, hygiene supplies, and disinfectants stored in the garage. LPA reviewed the facility's Fire / Earthquake Drill log noting their last fire drill conducted on 03/02/2026. Staff and client files reviewed had the required documentation. The facility safeguards the cash resources for three (3) of four (4) clients. LPA reviewed the Record of Client's/Resident's Safeguarded Cash Resources (LIC 405) for the three (3) clients while Administrator Mc Intosh counted their physical monies and no discrepancies were discovered. Facility sketch, clients' personal rights, and emergency disaster plan are visibly posted in the facility. Administrator Mc Intosh's administrator certificate expires on 07/09/2027. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Administrator Mc Intosh.
