The inspector’s account

On 10/03/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required inspection. LPA was greeted and entry by House Manager Cedric Williams, where LPA explained the purpose of the visit. The facility is licensed to serve (4) ambulatory clients, aged 18 to 59.

The facility was observed to be clean with the passageways being free of any obstructions. The food supply was observed to be sufficient as there was a 2 day supply of perishable and a 7 day supply of nonperishable food items. The medications, chemicals and sharps were observed to be locked and inaccessible to clients in care. The emergency disaster drills are being conducted on a monthly basis with the last drill being conducted on a monthly basis, with he last drill being conducted on 10/01/25.

A records review review was conducted of both staff and client files. Staff present were observed to have criminal record clearance and were associated to the facility. The staff were observed to possess CPR/First aid certification, and Crisis Prevention Intervention training. The administrator Lester Bell was observed to possess a valid administrator certification that expires on 03/15/27. The client files were observed to have admissions agreements, Individual Program Plan, and medical assessments.

The hot water temperature was tested and found to be within regulatory limits, measuring at 119.8 degrees Fahrenheit. The smoke and carbon monoxide detectors were tested and found to be operable. There are no pools or bodies of water, or guns and ammunition on the premises. Based on today's inspection there were no citations issued.

An exit interview was conducted where a copy of this report was reviewed and provided to Cedric Williams, House Manager.
