Facility Evaluation Report

Licensing Program Analyst (LPA) Seo Jeon conducted an announced case management visit. Upon entry, LPA was greeted by Alicia Diaz, Lead RBT, and informed them of the purpose of the visit. At the time of the visit, there were two (2) residents present. LPA also met with Heather Cobb, Behavior Specialist II from Department of Developmental Services who was conducting their semi-annual review.

LPA toured the interior and exterior of the facility. LPA reviewed records and observed that the Administrator was on leave since February 2025. This facility did not have a certified administrator.

A deficiency was cited per California Health & Safety Code and Code of Regulations, Title 22, Division 6.

An exit interview was conducted where a copy of this report was discussed with along with LIC809-D and Appeal Rights.
