Facility Evaluation Report

On June 24, 2026, Licensing Program Analyst (LPA), Venus Mixson arrived at the facility unannounced to conduct the Annual Inspection and met with the Lead Caregiver, Kayahna Walker. The facility is licensed for four Adults and is currently operating at a capacity of three Adults. For a 735 facility type.

LPA Mixson toured the facility along with Caregiver and made observations pertaining to the Annual Inspection. LPA inspected the facility inside and outside. There were no obstructions or debris to the indoor or outdoor passageways at the time of this visit. Additionally, there were no bodies of water on the premises. The facility is a single-story home located at 1403 Reeds Way, San Jacinto, CA. 92582. The facility phone number is (951) 350-0354 and it is operable. LPA observed the residents’ bedrooms, and each was equipped with required furniture as per Title 22. LPA inspected facility bathrooms, and the hot water temperature tested within regulations. The bathrooms were clean, and the appliances were operating appropriately at the time of this visit. The facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers. The fire extinguishers were last serviced on 06/01/2026 and was initialed by service provide.

LPA observed the required postings and emergency phone numbers as required. Cleaning supplies and sharp items were locked and inaccessible to the residents in care at the time of this visit. There was designated storage space for Residents' and Staff files. The files were locked and inaccessible to residents in care at the time of this visit.
