Facility Evaluation Report
On 03/25/2026, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the conduct a required annual inspection. Administrator Latasha Stokes arrived to the facility to meet with LPA. The facility has a fire clearance to serve four (4) non-ambulatory adults. During today's visit, LPA observed one (1) staff and zero (0) clients present. LPA was informed clients were out in the community attending day program. Staff present had a criminal record clearance and was associated to the facility.
LPA toured the facility with Administrator Stokes and observed the facility is made up of a one (1) story home with four (4) client bedrooms, two (2) client bathrooms, a kitchen, dining room, living room and attached garage. Indoor and outdoor pathways were free of obstruction. No bodies of water were observed on the premises . Client bedrooms had the required bedding, furniture, and lighting. Bathrooms had nonskid mats in the showers. The facility had a two-day supply of perishable foods and seven-day supply of non-perishable foods. Medications are centrally stored in a kitchen cabinet and secured with a lock. Cleaning solutions and disinfectants are secured in the locked garage. Administrator tested one (1) of the smoke alarms/carbon monoxide detectors and LPA heard it to be operational. LPA also observed a charged fire extinguisher mounted in the facility that last serviced on 06/04/2025. LPA reviewed the facility's Disaster Drill Form noting their last fire drill was conducted on 03/02/2026 and earthquake drill on 03/03/2026. Client files reviewed had all the Departmental required records. LPA reviewed t he Record Of Client's/Resident's Safeguarded Cash Resources (LIC 405) w hile administrator counted their monies and no discrepancies were discovered . House rules, clients' personal rights, complaint procedures, emergency disaster plan, and facility sketch are visibly posted near the front entrance. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Administrator Stokes.
