LPA LaVette Farlow attempted to conduct the required annual inspection visit. LPA rang door bell and knocked on door 3x with no answer. LPAs called phone numbers and left a spoke with Licensee. A staff member arrived during this time and expressed she left her key. Staff member stated someone was would be en route to open the facility. LPA L. Farlow waited and the staff that was present left to get the keys. LPA will return another time to complete the visit. 1:55pm-2:57.
