Facility Evaluation Report
On 6/1/2026, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required visit. LPA Flores was greeted by Administrator Brent Drouillard and explained the purpose of the visit. LPA Flores was granted entry into the facility where a tour was conducted alongside the Administrator. During the tour, LPA Flores observed the following:
The facility is a single-story structure which consists of (4) four client bedrooms, (3) three bathrooms, office, kitchen, dining room, (2) two-living rooms, laundry room, garage, and secured backyard. Client bedrooms were equipped with the required bedding, furniture, and functional lighting. Bedrooms were large enough to allow for easy passage and comfortable use of any required client-assistive devic es. Toilet, washbasin, bath and shower fixtures were observed to be in good repair. Water temperature measure within the required limits. The kitchen was observed to be organized and sanitary. LPA observed a (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. Knives and other sharp items were observed to be store in a locked cabinet. Emergency food and water were observed to be store in the garage and in sufficient in supply. Laundry services are provided on-site. Laundry machines were observed to be in operable condition. Detergents and other cleaning solutions were stored in a locked cabinet, inaccessible to clients. LPA observed the backyard to be furnished to promote outdoor activities. Carbon monoxide and smoke detectors were tested and observed to be fully operable. The facility maintain a large selection of activities such as board games and puzzles. Medication was observed to be centrally stored and inaccessible to unauthorized representatives. Per Administrator, there are no firearms and/or ammunition on the premises.
