Facility Evaluation Report
Licensing Program Analyst (LPA) Imaculada Vasquez conducted an unannounced required annual visit. LPA met with Manager Dlamini Khumbuzile who was informed of the purpose of the visit. At the time of the visit there was two (2) staff and one (1) client present. Two (2) clients were at day program during the time of the visit.
The facility is a one story home with client (5) bedrooms, (2) bathrooms, attached garage, and an outdoor space that has a shed that stores gardening supplies. The facility does not have a pool or firearms. The facility is a residential care facility for adults ages 18 to 59, approved for (6) ambulatory clients.
Physical Plant: Physical plant, floors, windows, and doors were observed to be clean and fixtures and furniture were present and in good repair. The facility's outdoor area was observed to be free of hazards. Laundry equipment was observed to be in good working condition. LPA observed locked centrally stored medication, sharps, and cleaning supplies. Carbon monoxide, smoke detectors and fire extinguishers charged on August 11,2025 were operational. Hot water temperature was recorded at 109.7F in the kitchen. The facility has cleaning supplies to conduct regular cleaning of the facility, and personal hygiene supplies for the residents.
