Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced visit to the facility for a required annual inspection. LPA was met with Administrator, Marvin Taylor who was informed of the purpose of the visit. There were (1) staff and no clients present during the visit. (1) client later arrived for the exit interview.

Facility is a two story home with (2) client bedrooms and (2) client bathrooms, there are staff bathrooms and bedrooms in the home. The facility has no pool and no firearms are being stored.

LPA observed the physical plant floors doors and windows, fixtures and furniture were in good repair and were present. There are extra linens and hygiene items, for clients and cleaning supplies to ensure the facility is sanitary. Facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. Facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods. The hot water temperature was recorded at 115.5F.

(2) Client files and (3) staff files were reviewed which possessed all required documents. All staff present are fingerprint cleared. The client personal and incident funds and ledgers were reviewed, funds were accounted for.
