Facility Evaluation Report
Licensing Program Analyst (LPA) Imaculada Vasquez conducted an unannounced annual required visit. LPA met with Staff, Innocent Onyema Okwudifele who was informed of the purpose of the visit. The administrator and licensee, Chyke Ehimere was available over the phone. At the time of the visit there was (1) staff and zero (0) clients.
The facility is a two story home with (4) bedrooms for clients and (3) bathrooms with attached garage. No pools or firearms are being kept at the facility. The facility is an adult residential facility licensed for (6) ambulatory clients. Although they have no clients residing at the facility.
Infection Control: The LPA observed hand washing stations, hand hygiene supplies, personal hygiene supplies, PPE equipment, and cleaning supplies to do regular cleaning of the facility. LPA reviewed the facility's infection control plan which met department requirements.
Physical Plant: Physical plant, floors, windows, and doors were not in a clean state. LPA observed the entire facility in disarray. The front yard has a sprinkler leak. LPA observed cleaning crew arrived while at visit. Staff reported repair man is on the way. A deficiency was cited and plan of correction was created. The sharp and dangerous objects were observed to be locked and inaccessible. The smoke detector and carbon monoxide was operational, and the hot water temperature 119.8F.
Food Service: LPA observed facility kitchen had the ability to prepare food and possessed equipment in good working condition. LPA observed the facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods.
