Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced case management visit to deliver an amended LIC809 report. LPA was greeted by staff, Gabriela Alvarez, and explained the purpose of the visit. LPA conducted a tour of the exterior/interior of the facility and did not observe any health and safety concerns.
LPA generated a case management report under the incorrect facility number. Corrections are set to be made.
