Facility Evaluation Report

Licensing Program Analyst (LPA) Jesse Gardner, conducted an unannounced visit to the facility to initiate investigations into complaints (18-AS-20230809151322) and (18-AS-20230810085421).

LPA met with Administrator Alexander Ogle and informed him of the purpose of the visit. LPA then toured the facility. During a review of employee roster, LPA found that Staff 1 (S1) does not have a background clearance in Guardian.

During interview with S1, S1 indicated that they had been working for the facility since April, 2023 as a counselor for the clients. This is a violation of Title 22 CCR 80019(e)(1). Facility cited, and an immediate civil penalty was assessed. LPA later observed S1 leave the facility property.

An exit interview was conducted where a copy of this report was discussed with and provided along with copies of the LIC811, LIC421BG, and Appeal Rights.
