Facility Evaluation Report
On 03/26/2025 Licensing Program Analyst (LPA) Raquel Hernandez conducted a case management visit at facility. According to Complaint 56-AS-20240319145136. Administrator Ghislaine and Oscar Ramasar failed to report to the department Resident #1 (R1) leaving the facility. within 7 seven days of the occurrence. R1's physician report states they are unable to leave the facility without supervision.
Based on the observations made during today’s visit, a deficiency was cited per Title 22, Division 6, of the California Code of Regulations.
An exit interview was conducted, and this report (LIC809) (LIC809D) was discussed and provided to Assistant Administrator Ghislaine and Oscar Ramasar. Along with a copy of the appeal rights.
