Facility Evaluation Report

Licensing Program Analysts (LPA) Janira Arreola and Abdoulaye Zerbo conducted an unannounced visit to the facility for a case management visit. LPAs met with Administrator, Cotadela Balatbat, who was informed of the purpose of the visit. At the time of the visit there were (3) staff and (4) clients at the time of the visit.

LPAs conducted a tour of the facility and conducted a health and safety check on the facility clients. No immediate health or safety issues were observed.

LPAs conducted a visit in response to a self reported incident for Client #1 (C1). LPAs conducted interviews and records review.

At this time no deficiencies were cited, and additional information may be needed for the reported incident. An exit interview was conducted with Administrator, Cotadela Balatbat where this report was reviewed and provided.
