Facility Evaluation Report
Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced required annual visit. LPA met with Administrator, Jeffrey Castillo, who was informed of the purpose of the visit. At the time of the visit there was (5) staff and (6) clients present.
The facility is a one story home with (4) client bedrooms, (2) bathrooms, attached garage, and an outdoor space. The facility does not have a pool or firearms. The facility is a residential care facility for adults ages 18 to 59, approved for (6) non ambulatory clients.
Physical Plant: Physical plant, floors, windows, and doors were observed to be clean and fixtures and furniture were present and in good repair. The facility's outdoor area was observed to be free of hazards. Laundry equipment was observed to be in good working condition. LPA observed locked dangerous items, such as centrally stored medication, sharps, and cleaning supplies. Carbon monoxide, smoke detectors and fire extinguishers were charged and operational. Hot water temperature was recorded at 108F in a client bathroom. The facility has cleaning supplies to conduct regular cleaning of the facility, and personal hygiene supplies for the residents.
