Facility Evaluation Report
On 01/29/2026, Licensing Program Analyst (LPA) Janette Romero made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Administrator Andres Amante who was informed of the purpose of the visit. The facility has a fire clearance to serve six (6) non-ambulatory adults ages 18-59. During the visit, LPA observed four (4) staff present. LPA was informed all clients were out in the community or attending day program. LPA conducted a record review and all four (4) staff present have a criminal record clearance and are associated with the facility.
LPA conducted a tour with administrator and observed the home is made up of three (3) client bedrooms, three (3) bathrooms, two (2) dining rooms, a kitchen, living room, great room, staff room, and attached garage. Indoor and outdoor passageways are free of obstruction. The facility has an in-ground pool in the backyard that is gated and secured with a master lock. Client bedrooms had the required bedding, furniture and lighting. Bathrooms had toilet paper, soap, and paper towels readily available along with grab bars and non-skid mats in the shower. LPA toured the kitchen and observed the facility met Departmental requirements for a two-day supply of perishable foods and seven-day supply of non-perishable food items. Medications are secured in a locked kitchen cabinet. The smoke alarm and carbon monoxide detector were tested and LPA heard them to be operational. LPA also observed a fire alarm pull station and a charged fire extinguisher mounted in the kitchen that was last serviced on 03/07/2025. LPA reviewed the facility's emergency disaster drill logs noting their last fire drill was conducted on 01/01/2026 and earthquake drill on 01/02/2026. LPA reviewed the Record Of Client's/Resident's Safeguarded Cash Resources (LIC 405) for all six (6) clients while the administrator counted their physical monies and no discrepancies were discovered. Clients' personal rights, facility sketch, and emergency disaster plan are visibly posted near the front entrance. During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Administrator Amante.
