Facility Evaluation Report
On September 11, 2025, Licensing Program Analyst (LPA) Venus Mixson arrived unannounced to the facility to conduct a Case Management visit on the Health, Safety, and welfare of residents in care, and spoke with Licensee Caroline Sibande. LPA was informed three residents currently reside at this facility, and are present, along with three staff. The Licensee was available by phone at the time of this of the visit.
LPA Mixson toured the facility and observed the facility utilities to be on and operable at this time. The facility food supply is sufficient, the facility is clean and free of clutter and/or debris. The doorways are clear, required posting per regulations are present and observed at the time of this visit. LPA did not observe any immediate concern for residents in care. Additional information was requested and obtained. LPA interviewed the Licensee, three staff, and the three residents present at the time of this visit.
No deficiencies were observed or cited. No civil penalties per California Health & Safety Code and Code of Regulations, Title 22, Division 6, were observed or cited at this time.
An exit interview was conducted with Caroline Sibande and a copy of this report was discussed and given to Facility Staff.
