Facility Evaluation Report
Licensing Program Analyst (LPA) Sara Martinez conducted an unannounced Required Annual Visit. LPA was granted entry and met with Licensee Victor Ugarte who was informed of the purpose of the visit.
LPA was informed that currently there are no clients placed at the facility. The facility is currently undergoing construction due to water damage. Licensee stated the facility has been waiting for the insurance company to begin construction. Insurance company has approved the construction and construction of the house will begin Monday October 16, 2023. LPA conducted a tour of the facility's interior and part of the exterior and observed the following:
The physical plant of the facility is uninhabitable due to the water leak. LPA noted drywall removed, exposed framing, windows, doors, and walls are broken and need repair. The facility has electricity and water. Due to the water leak the facility did not have any flooring. The facility did not have any fixtures and furniture in client's bedrooms, living room area, and dining room. The kitchen did not have any cabinets, sink, or appliances. The bathrooms did not have any appliances. The outdoor area was hazardous and had broken furniture. The front door and back door cannot be locked due to doorframe damage.
Licensee was unable to test the smoke and carbon monoxide detectors due to being removed for construction therefore LPA was unable to determine if smoke and carbon monoxide detectors are operational. The facility did not have a fire extinguisher present. Licensee was unable to test the hot water temperature due to no sinks being present at the facility.
Based on LPA's observation, the facility is not ready for client placement. Although there are no clients currently residing in the facility, the issues and concerns observed by LPA must be fixed prior to accepting any clients. In addition, LPA advised Licensee that the facility must call the Community Care Licensing prior to accepting clients.
LPA issued (2) Technical Violations for the condition of the facility, as well as the debris inside and outside the facility. An exit interview was conducted where a copy of this report, and both LIC9102's were discussed and provided to the Licensee.
