Facility Evaluation Report

Licensing Program Analysts (LPAs) Janira Arreola and Seo Jeon, conducted an unannounced visit to the facility in order to conduct a Plan of Correction (POC) visit. LPAs met with staff, Maribel Aguilar during the visit. LPAs conducted interviews and walk through during the time of the visit. During the time of the visit no clients were present and (1) staff was present.

04/27/2026 LPAs conducted a Case Management Visit and issued a deficiency for 80087(a) Buildings and Grounds. The POC was for the licensee to replace the smoke alarms and provide proof of correction to the LPA by the POC due date of 05/01/2026.

During today's visit LPAs observed the smoke alarms were replaced and were tested and operational.

LPAs also conducted a health and safety check, observed food supply and working utilities. No other health or safety issues were observed during the time of the visit. The Staff was provided with an exit interview were this report, along with clearance letter were provided.
