Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola, conducted an unannounced visit to the facility in order to conduct a case management visit. LPAs spoke to Licensee, Mary Martin over the phone during the visit. LPA met with Staff, Marlene Martin, who was informed of the purpose of the visit. LPA conducted interviews and walk through during the time of the visit.

During the time of the visit there were (1) client and (1) staff present. LPA observed client rooms, food supply, utilities, and criminal record clearance for staff. LPA observed no immediate health or safety issues during the time of the visit. An exit interview was conducted where this report was reviewed with the staff. A copy of this report was provided.
